TERMS AND CONDITIONS

Customer will be responsible for the following: 

1. Education of appropriate personnel on federal, state, local and Joint Commission requirements for the  handling, storage and tracking of human tissue for transplantation.
2. Documentation of receipt of each Product unit by serial number and track its location within Customer’s  facility.
3. Documentation of Product usage information in an implant logbook to clearly identify the specific unit  being implanted into a specific recipient.
4. Providing Product recipient information back to the applicable tissue bank utilizing the Tissue Transplant  Records provided with each Product unit.
5. Reporting Product complaints and adverse outcomes potentially attributable to this tissue Product must  be reported promptly to SUZZOR via email at CustomerService@suzzor.com.
